  • Be entitled to Medicare Part A and be enrolled in Medicare Part B
  • Live in our geographic service areas

  • Have one of the plan’s specified chronic conditions:
    • Diabetes Mellitus
    • Chronic Heart Failure
    • Cardiovascular Disease
  • Be entitled to Medicare Part A and be enrolled in Medicare Part B
  • Live in our geographic service areas
  • Be a United States citizen (or lawfully present in the United States)
